The Centre for Population Health (CePH) is pleased to share that Dr Kwong Soke Chee, Research Officer at CePH, has been awarded the Young Investigator Award (Early Career Research Category) at the UMHealth Academia Month & Research Carnival 2025 Symposium, happening on 14 – 16 October 2025. This recognition reflects Dr Kwong’s dedication to advancing patient-centred digital health solutions and her strong commitment to driving meaningful innovation and rigorous research that contribute to improved health outcomes.
Dr Kwong presented the study titled “Experience of Patients and Caregivers on Using a Digital Personal Health Record for Lifelong Congenital Anomaly Care.” The project centres on the CARE Passport, a mobile application designed to support patients and caregivers in managing lifelong follow-up for congenital anomalies. By improving access to personal health information, simplifying record organisation and enhancing continuity of care, the platform aims to strengthen coordination across clinical teams and empower families navigating complex health needs.
The study represents a collaborative effort led by Principal Investigator Dr Tan Yew Wei, together with co-researchers Dr Tiong, Dr Suganthi and Prestinne, who contributed to the development, refinement and evaluation of the digital tool. Their combined expertise in clinical care, digital health design and patient engagement has been integral to shaping the project’s overall direction and impact.
In parallel, efforts are underway to further translate the CARE Passport into broader use. Dr Tan has established a UM spin-off company focused on advancing and scaling the platform, supporting its progression toward real-world implementation. This step is expected to facilitate wider accessibility for patients and caregivers who may benefit from a structured, connected and user-friendly approach to lifelong congenital anomaly care.
