Is my data safe?
All data (including name, contact number, address, questionnaire responses, and medical results) are kept strictly confidential. We will never release individual information about any participant to anyone for any reason. Only staffs who perform updates to the file or follow-up mailings have access to the name and address file. Each participant of the cohort is assigned a unique ID, anonymous data will be used for analyses.
Will I get any medical advice at the end of the survey?
We will not provide any medical advice, but we will highlight any abnormal results in your survey.
Can I continue the survey from where I stop?
Yes, please remember to click the “Save & Return Later” button at the end of the page. You will be asked to provide an email address where a unique link will be sent to your email address to continue the survey at a later time.
What if I lost my unique link to continue filling in the survey?
Please email us at clusterstudyum@gmail.com and we will send you the unique link to complete the survey.
When will I receive the certificate of participation?
A certificate of participation will be emailed within 2 weeks after completion of the questionnaire.
